OFFICE POLICIES AND RATES

The main objective of Dark Horse Chiropractic and Performance is to empower and educate the patient with premium, personalized care. We understand that in the current state of healthcare, it is easy to become a number in the system and “fall through the cracks” so to speak. As a patient, you can expect that your care is specific to you and your performance goals, with our focus being placed on providing you the means to get there.

Payment and Health Insurance: We do not accept or work with insurance or insurance companies. We are considered "out-of-network" for all insurance companies. You can find out more information about your out-of-network benefits by calling the customer service number on your insurance card to inquire about your chiropractic coverage. Depending on your insurance plan and coverage, a superbill may be provided to you so that you may get reimbursed directly from the insurance company.

We also accept Flexible Spending Account (FSA) and Health Savings Account (HSA) as forms of payment.

Otherwise, payment is due at time of service.

Automobile Accidents: Unfortunately, we do not work with Motor Vehicle Accident (MVA) cases. However, we do have a network of chiropractors that do work with MVA cases. We would be happy to provide recommendations.
